Coach Application (Tweed Minor Hockey Association)

Please fill out all the information below. Forms must be complete to be considered. Successful applicants will be required to provide a police background check a minimum of 30 days before the season begins. Applicants must have Coaching/Trainer Level Qualifications to qualify as bench staff. Respect In Sport Activity Leader, Gender identity Expression Certification prior to season start. Please note that to be considered for a coaching position you must fill out the below application even if you coached the previous year. We are asking each applicant to submit two (2) professional references. We appreciate your interest in coaching for Tweed Minor Hockey Association; successful applicants will be offered an interview.
